Your privacy matters. The conversation checklist does not send or save your selections. When you submit the health questionnaire with both consent boxes checked, all ten answers, including any health information you enter, are processed by this website and emailed to Hailey Khashan at haileykhashan@gmail.com for your insurance inquiry. A separate confirmation email to your supplied address contains no answers. Our application does not store answers in a database, browser storage, URLs or analytics, or deliberately write them to logs. Email providers and authorized mailbox users process and retain the messages. Contact Hailey at (678) 451-5546 about access, correction, deletion or another way to share information; deletion may be subject to applicable recordkeeping obligations. BotID processes request/browser signals to protect submissions. Email is not a guarantee of confidentiality. No coverage or appointment is created by submitting. The review draft is not sent or saved by this website. We do not add advertising cookies or analytics to the site. The embedded Calendly calendar has its own privacy practices and may use cookies to provide scheduling.

The hosting provider may process ordinary request information, such as IP addresses and browser information, to deliver and secure the website. Phone links open your device’s calling application. Facebook, Google, Calendly and linked official resources have their own privacy practices. Details entered in the Calendly calendar are handled by Calendly and the meeting organizer; consult its privacy notice before submitting.

Insurance applications may require personal information through the carrier’s secure application process. Please do not put Social Security numbers, medical records or payment details in a review, ordinary email or social message. Ask Hailey for the appropriate secure application channel.
