01

Premium and deductible

The premium is the amount you pay to keep coverage. A deductible is an amount you may need to pay for covered services before the plan begins sharing certain costs. Some services can be covered before you meet it.

02

Copay and coinsurance

A copay is generally a fixed amount for a covered service. Coinsurance is a percentage of the allowed cost. Which applies can depend on the service and whether you have met the deductible.

03

Out-of-pocket limit

For applicable covered, in-network services, this limit caps the cost sharing you pay during a plan year. Premiums, excluded services, and certain out-of-network costs are not included. Read your plan’s rules; not every type of insurance works the same way.
