01

Confirm activation and keep your documents

Check the effective date, payment requirements and member ID with the insurer. Keep the policy and benefit summary in a secure place. An application or conversation alone does not mean coverage has started. If something is missing, ask the insurer before relying on benefits.

02

Set up the correct member access

Use the portal and contact information on your policy or ID card. Hailey publishes client resources, including MyUSHG, provider lookup and telehealth information. These resources are relevant only when they apply to your actual policy; their presence on a website does not establish your eligibility.

03

Check before receiving care

Verify the exact provider network, any referral or authorization requirements, and your expected cost sharing. Check prescription coverage with the plan and pharmacy. Authorization is not itself a guarantee of payment; coverage depends on the policy and claim circumstances.

04

Keep bills and benefit explanations separate

An Explanation of Benefits describes how an insurer processed a claim; it is not the same as a provider bill. Compare the documents and contact the insurer or provider about discrepancies. Use secure member channels for claim details rather than public reviews or social messages.
